It reflects an experience many women have but rarely articulate: the inability to fully surrender to pleasure when the mind is still occupied by past harm.

 

The painting uses violent interruption as a metaphor: the woman's face suggests pleasure or release, but it's literally being "pierced" and disrupted by the "bullet" or the horrible flashback.
